Parental Consent Form
The Parental Consent Form is required by the National Council for Special Education (NCSE) to allow the NCSE to collect and use personal and professional information about a child in order to support their special educational needs.

Consent Form for Ukranian/Russian Translation of Professional Reports
If you would like to submit a request for translation of professional reports from Ukrainian/Russian to English, please submit your request to translations@ncse.ie.
When submitting the request, please attach the report for translation and a signed Parent Consent Form. The forms are available here in English and Ukrainian as needed.
Translation requests will not be accepted without the signed Parent Consent Form.

School Transport
The NCSE processes applications for school transport for students with special educational needs. Completed application forms should be submitted to your SENO along with the professional report(s) supporting the application. Applications are forwarded by the SENO to the School Transport Section in the Department of Education, who make the decision on all applications.
