By filling in this form, you agree to allow the NCSE to collect and use personal and professional information about your child to support their special educational needs.

Consent Form for Ukrainian/Russian Translation of Professional Reports
This form is usually filled out by the school principal and signed by the parent or guardian.
To submit a request for translation of professional reports from Ukrainian or Russian to English, please email translations@ncse.ie
Please attach two documents:
– The report for translation
– A signed Parent Consent Form (link below)
The forms are available here in English and Ukrainian.
We cannot accept translation requests without the signed Parent Consent Form.

Consent Form for Ukrainian/Russian Translation of Professional Reports – Home Tuition
This form is usually filled out by the parent or guardian when seeking the Special Educational Needs Home Tuition Grant Scheme.
To submit a request for translation of professional reports from Ukrainian or Russian to English, please email translations@ncse.ie.
Please attach two documents:
– The report for translation
– A signed Parent Consent Form (link below)
The forms are available here in English and Ukrainian.
We cannot accept translation requests without the signed Parent Consent Form.
School Transport
If you are a parent or guardian seeking school transport for your child, please contact your local special educational needs organiser (SENO).
Your SENO can co-ordinate the application. The form has sections to be completed by:
– The parent or guardian
– The school
– The special educational needs organiser (SENO)
Completed application forms must include the professional report(s) supporting the application.
The SENO will send the applications to the School Transport Section in the Department of Education, who decide on all applications.
